Abstract

This document analyses structural, cognitive, behavioural, political, and cultural failure modes that compromise AI deployment and provides an integrated Neya Risk Architecture to prevent them. It reframes AI failure not as a technological malfunction but as a systemic breakdown rooted in human environments.

The analysis identifies predictable failure patterns: distorted incentives, unclear governance, emotional reactivity, institutional fragmentation, misaligned expectations, cultural resistance, and the absence of deployment clarity. It introduces risk categories, architectural safeguards, control points, and prevention loops that must be embedded before and during deployment.

This is the core risk-diagnostic instrument of the AIDA discipline, suitable for any institution attempting to roll out AI in complex human systems.
